This conference was an opportunity to examine next steps for the integration and delivery of care services for older people.
Delegates discussed how outcomes across primary, secondary and domiciliary care can be improved, funding challenges following the implementation of the cap on care costs introduced through the Care Act; and progress in the Department of Health and NHS England's £400m programme for improving care for vulnerable people, including GP responsibility for out-of-hours care and the delivery of individual care plans.
