Following the launch of the Department of Health and NHS England's £400m programme for vulnerable older people, which made named GPs responsible for out-of-hospital care around the clock and offered individual care plans, debate focused on how outcomes across primary, secondary and domiciliary care can be improved and the challenges of personalisation. It took place with the Care Bill - which suggested changes to funding - in Parliament.
